2023 Summer Art Camp Form
In the event that you cannot be reached, please list 2 alternative adult contacts that have your permission to pick up your child/children and we must have all telephone numbers: Cell, Work, Home. A $40 fee per hour and $10 per 15 minutes will be charged for late pick-ups after 5:30. ID will be required to release your child. If you have an emergency and cannot pick up your child on time, please notify the HAL Manager at 863-385-5312 or Camp Director, Marcia Davis, at 863-632-0202.

While the Highlands Art League and its instructors make every effort to provide a safe learning environment, I understand that there is always the risk of an accident. I will be responsible for any medical or other costs incurred resulting from illness and/or claims or damages arising out of personal injury of any kind. In the event of any medical emergency, I authorize the Highlands Art League to administer minor first aid and/or to seek emergency medical treatment for my child including calling 911.
If a child is consistently aggressive towards other children (including hitting, pushing, biting, etc.), or disruptive in the learning process, Highlands Art League reserves the right to ask you to remove your child on that day, or, in extreme cases, for the remaining week or weeks. A refund will NOT be given for the remaining time that has been paid.
*Students will be required to wear toe-closed shoes for outside activities and safety.
*NO toys of any kind from home are allowed.
*Students may get paint, glue, glitter, etc. on their clothing. Play clothes recommended.
*Students may bring their own snacks for snack time.
Releases: Agreeing to terms and conditions does hereby release and forever discharge Highlands Art League, Inc. and the City of Sebring from any and all manner of actions, suits, damages, claims and demands whatsoever in law or equity from any loss or damage to the undersigned’s property or person during his/her participation in and presence on the premises of the Highlands Museum of the Arts and the Art League’s Visual Arts Center, Sebring, FL.
Photo Release: Agreeing to terms and conditions does hereby grant permission to THE HIGHLAND’S ART LEAGUE, INC., its agents, and its employees the irrevocable and unrestricted right to produce photographs and video taken of my child, myself, and members of my family while at Summer Art Camp at HAL for any lawful purpose including publication, promotion, illustration, advertising, trade, or historical archive in any manner or in any medium. I hereby release THE HIGHLAND’S ART LEAGUE, INC. and its legal representatives from liability for any violation or claims relating to said images or video. Furthermore, I grant permission to use the statements of my child, myself, or my family members given during an interview or evaluation with or without my name for the purpose of advertising and publicity without restriction to time limit or geographic area. I waive my right, my child’s rights, and my family’s rights to any and all compensation stemming from the use of these materials.
$50 returned check fee, and any other acquired costs. Payment is due before 5pm the Friday prior the the first day of the selected week. Payment online or in person with Square and credit card or check made to Highlands Art League, Inc.
